👍👍 – I, the undersigned {Khalid Mahmood} OW recipient, ready to submit the affidavit in a dispute with the housing worker of the shelter that I have complied by all the requirements of Ontario Works as per the details in the affidavit (i.e. before submitting a second set of CPP and OAS applications, if required by the Ottawa office of OW in writing, I need it in writing for my protection in future.). Affidavit was sworn and witnessed by a City of Ottawa official at one of the Client Services Centres on June 26 of 2023 around 10h20 am. Paid a fee of $15.71 for it by cash.

TO WHOM IT MAY CONCERN/INTEREST

In Brampton (Ontario) OW social services was delivered by the Human Services Department of Peel Region, located inside the Peel Region building. Peel Region building was located next to Police Head quarters of the area, with mall and the bus terminal on the opposite side of the building.

(Housing worker used the word non-compliance) for something which was already completed by me 11 months ago in August 2022 nearly a year ago in a different municipality, in the municipality of Brampton which is located in the Region of Peel in Ontario and is part of GTA as well.

I would also like to mention that I will file my taxes to revenue Canada as soon as I owe taxes, at present I have no other income (=zero dollars). Only money I get is from OW social assistance (which is an incomplete cheque for the past several months). Also to clarify that I am not a tax collector and I am NOT collecting GST or any other taxes. This is not a non-compliance.
