To the attention Overpayment Recovery Department and Mr. Bill Petit of City of Ottawa,

(Social Insurance number xxx-xxx-766).

My Ontario works file was closed on September 30 of 2023. After the file was closed I received a letter regarding over payment (which never occured).

I would like to have a in person meeting with Ms Vicki Brasil who met with me at the end of June 2023 and early July 2023 to discuss this matter when my file was transitioning from OW social assistance to OAS.

At that time, I was asked to sign three document i.e.

agreement to reimburse and deduction schedule etc. Which I complied with and signed without consulting with a lawyer. This was in addition to the monthly amount deducted from my cheque every month.

OAS has deducted an amount from my OAS payment in September 2023.

I need you to provide me an updated status of overpayment and acknowledge the deductions made by OAS towards overpayment.

Please treat this as urgent.
